The funny thing is I haven’t touched this form since May 18th.

I am under the impression that this post is not supposed to be alone in this topic. (it seems to quote some other message and not give too many other helpful details)…
Could you please get me out of the dark by either giving me a link to some other topic or by adding some details of the problem? (like what exactly happened and what triggered it, what is the problem and is it really solved - as the quote states?)

I believe the quoted text is an error message put out by Servoy when two forms have the exact same name. I have seen this in the past following the import or update of a solution. I my case I deleted the duplicate form and all was well, but it may be best to hear a response from Servoy before taking such action yourself. In my case I was also certain that I had not created the duplicate form.
